Both Student & Parent/Guardian must read & agree to each section of the Extracurricular & Social Eligibility Code, and sign at the bottom of the form to indicate you have read and understood all parts of the Extracurricular & Social Eligibility Code, Assumption of Risk, Locker Room Rules & Hazing Policy. This form needs to be completed in full in order to participate in all CV sports, Marching Band, Cheerleading, Fall Play, Spring Musical, GHMS Spring Play, Indoor Color Guard & Indoor Percussion and must be signed/acknowledged each season/activity. (Please fill out a new acknowledgement form for each student).
