Necessary Documentation

To register a new student, Pennsylvania law requires* you to present specific documentation during the registration process.

What Documentation is Required?

For NEW Students

NEW students enrolling into Conestoga Valley School District from another school district, private school cyber/charter, home education, attending school for the first time, the following is required:

  • Student's birth certificate or verified age document
  • Parent/Guardian ID
  • Student's immunization records
  • Deed/mortgage/bank related loan statement or lease agreement
  • ONE of the following (less than 3 months old):
    • Utility bill
    • Internet bill
    • Credit card bill
    • Bank statement
    • Tax bill
    • Property insurance document; or
    • Federal or state government mailing such as benefits information, voter registration, or tax documents

For CURRENTLY REGISTERED Students

CURRENTLY REGISTERED students who move to another household within the Conestoga Valley School District, the following is required:

  • Deed/mortgage statement or lease agreement
  • ONE of the following within 30 DAYS of the household address change:
    • Utility bill
    • Internet bill
    • Credit card bill
    • Bank statement
    • Tax bill
    • Property insurance document; or
    • Federal or state government mailing such as benefits information, voter registration, or tax documents

For Incoming Grades K/1, 6, 11

  • Physical exam form signed by medical practitioner

For Incoming Grades K/1, 3, 7

  • Dental exam form signed by dentist

If Applicable

  • Custody agreement
  • PFA order
  • Affidavit of guardianship
  • Foster care placement letter
  • Doctor's orders (physical and/or milk restrictions)
