Address Change

CVSD requires notification when a student's address has changed. A Student Address Change form should be submitted by following the steps below. Note: The address change process is completed at the building level.

How to Change Your Student's Address

In Person

Please visit the main office at your student's school to complete the Student Address Change form. Note: Proof of residency documents are required to change a student’s address.

Online

  1. Log into your parent PowerSchool account
  2. Navigate to "Forms"
  3. Click on and complete the "Student Address Change" form

Note: Proof of residency documents are required to change a student’s address. These documents may be submitted digitally in PowerSchool or can be submitted at your student's school.
